TO ENSURE THE VACCINE POLICY HAS BEEN UNDERSTOOD AND FOLLOWED, THIS AGREEMENT IS REQUIRED TO BE FILLED OUT IN ORDER TO BOARD AT OUR FACILITY.

By clicking this box, you understand that if you choose to vaccinate your pet for kennel cough (not required), it MUST be done at least 4 weeks prior to their stay. We have the right to refuse boarding for the safety of other pets in our care if done less than 4 weeks.

By clicking this box, you understand if your pet presents kennel cough symptoms while in our care, pick up arrangements must be made. Failure to do so will result in an additional quarantine fee.
